Flex Adapter Focus

Have a Leaf Aptus IIwith a Mamiya mount on a Flex Adapter for a Cambo. Does the ground glass on the Flex Adapter have to be changed with various back adapters? The problem is that focus is not aligned between the two. The ground glass appears to be 3 or 4 millimeters different from the level of the sensor. As you can imagine, it makes focusing a challenge.

The Flexadapter which is a Phase One solution does not require any change between back mounts.
The insert for the specific mount takes care of this.
Since there is very few mechanical parts on the adapter it sounds more likely that the back be off.
That should of course be verified with another camera.
