Same shutter speed, different exposure

I have a Mamiya 645DF+ with Credo 40 Back. All works well, and will be on a job, shooting away, and then all of a sudden, with exactly the same camera settings, a frame will be a lot darker, like the shutter has been turned up three stops.

All the camera information is still the same on that file as the previous one (with correct exposure), and there is a picture there, but I am suspecting some kind of shutter error. The next frame it might go back to how it should be, and then the following back to dark. There doesn't seem to be a pattern to it.

Wondering if anyone else has experienced this same problem. I tried searching the forum, but, it's hard to know what key words to input for this. If someone has a link to another topic, happy to see it.

Sounds like a shutter issue to me. Time for a visit to the lens doctors.
