Mamiya 645 AFD II + Sekor 80 1.9 N

Hi,
I recently bought my mamiya. I get fast lens for it and i start taking a photos. it is my first medium format camera so i have no knowledge other than from net. However I'm professional photographer but never working on medium format.
My problem is, all my photos are out of focus. i did 8 rolls of films so far, scan them in Uk Film Lab.
last 2 rolls i did test : i focus on static object with camera on tripod and using a focus light confirmation, i use aperture from 1.9 , 2.8 , 4.5 to make sure i will get sharp photo.
And even with this all photos are out of focus.
Lab said:
'You appear to have an issue with film flatness. You’ll notice that the areas of focus are in a number of planes rather than one distinct plane. You might want to check your film inserts – the reels should only spin in one direction"
Film insert is brand new, and looks like is fine.
I read that add II does not work great with sekor 80 1.9 manual lens ?
I asked mamiya support before i bought it and they said it will be working, but no Af and can not change aperture on dial etc..
NOt sure what to do now, anybody any issue with this duo?
Is there any service where i can send it for check?
Thanks for help.
